How the program works

From referral to durable placement in three steps

Every participant enters through the same path: a trusted referral, a matched role paired with housing navigation, and a 90-day follow-through window that protects the placement from falling apart.

    A shelter partner, employer, or community organization refers the participant through a short intake. We only ask what we need to begin — nothing clinical is required to get started.

    Match & place

    We pair the participant with the right transitional role and a dedicated housing navigator using a Housing First approach. Income begins flowing while the housing plan takes shape.

    Step

    Follow-through

    We stay with the participant for 90+ days post-placement — transportation, conflict resolution, housing stability. Placement that survives the first quarter is the outcome that counts.
